Does Exercise Cause Hair Loss? Myths About Male Hormones and DHT | Yeoksam-dong Hair Loss Clinic

Does weight training cause hair loss? This article explains how testosterone, DHT, anabolic steroids, creatine, and crash dieting really relate to male pattern hair loss.

Hello, I am Kim Da-jeong, Chief Director of Cordia Clinic.

Some people start to worry about hair loss when they begin going to the gym or doing strength training.

Recently, during consultations, I have been asked whether it is okay to exercise while taking hair loss medication.

"Does weight training raise male hormones and make you lose hair?"

"If I build more muscle, will my hair loss get worse?"

To answer up front, there is little medical evidence that ordinary strength training directly causes male pattern hair loss.

Testosterone can rise temporarily after exercise, but that does not necessarily lead to hair loss.

Today, I will take a medical look at the relationship between exercise, male hormones, and hair loss.

1. Will high male hormone levels cause hair loss?

Male pattern hair loss develops from a combination of genetic predisposition and the effects of male hormones.

The most important substance here is "dihydrotestosterone (DHT)."

DHT is produced when testosterone is converted by the enzyme 5α-reductase.

Diagram: 5α-reductase turns testosterone into DHT, and DHT-sensitive follicles shrink: thinner, shorter hair and lower density.

In hair follicles that are genetically sensitive to DHT, the growth phase of the hair shortens and the follicles gradually shrink, a process known as 'follicular miniaturization.'

As a result, thicker hairs become finer, and hair density decreases at the crown or the front hairline.

But there is an important point here.

Male pattern hair loss is not simply a condition caused by high testosterone levels. What matters more is how sensitively hair follicles respond to androgens.

In fact, people with normal blood levels of male hormones can also develop male pattern hair loss.

2. Does strength training increase testosterone?

A systematic review published in PeerJ in 2026 analyzed 15 randomized controlled trials involving a total of 251 participants.

The study found a tendency for testosterone to rise temporarily after strength training and after moderate- to high-intensity exercise.

However, these changes varied depending on exercise intensity, type, and age, and in most cases testosterone returned to baseline during recovery.

In other words, a short-term rise in male hormone levels right after exercise should be distinguished from the long-term effects of androgens on hair follicles.

At present, there is insufficient consistent clinical evidence that hormonal changes from typical strength training worsen male pattern hair loss.

3. Are muscle-building steroids linked to hair loss?

Ordinary strength training should be distinguished from the use of anabolic androgenic steroids.

Taking high doses of male hormone-like drugs from outside sources to increase muscle mass can significantly alter the body's androgen environment.

In particular, people with a genetic predisposition to male pattern hair loss may experience accelerated hair loss.

The UK's NHS also lists hair loss among the major side effects of anabolic steroids.

Therefore, rather than exercise itself, attention should be paid to the effects of hormonal agents used to build muscle.

4. Does creatine, which many people take while exercising, cause hair loss?

Creatine is a widely used supplement for improving exercise performance and strength.

Earlier small-scale studies reported increases in DHT after creatine supplementation, which raised concerns about a link to hair loss.

However, a randomized controlled trial published in 2025 in the Journal of the International Society of Sports Nutrition reported different results.

In a 12-week comparison of creatine and placebo among 38 participants who completed the study, no significant differences were found in DHT levels, hair density, or hair thickness.

For now, it is difficult to conclude that typical creatine intake causes hair loss.

That said, more research on long-term use is still needed.

5. What if your hair starts falling out after you begin exercising?

If you started exercising and at the same time lost weight quickly or greatly cut back on how much you eat, the picture may be different.

Extreme calorie restriction and deficiencies in nutrients such as protein and iron can trigger telogen effluvium (resting-phase hair loss).

Telogen effluvium can also appear as heavy shedding about two to three months after the trigger.

So if hair loss has worsened since you started exercising, do not blame male hormones alone. Check your recent weight changes and nutritional intake as well.

In clinical evaluation, it is important to assess not only the amount of exercise but also the rate of weight loss, food intake, family history, and changes in hair thickness observed under scalp magnification.

6. Frequently Asked Questions

Q. Can I keep going to the gym if I have male pattern hair loss?

Yes. There is no need to stop ordinary strength training because of hair loss. However, adequate nutrition and recovery are important.

Q. Is aerobic exercise safer for hair loss?

There is insufficient evidence that aerobic exercise is more effective than strength training for preventing hair loss. It is best to choose exercise that suits your health condition.

Q. Does exercise always increase DHT?

No, it does not. A temporary rise in testosterone does not mean DHT will keep rising.

7. You don't need to give up exercise just because you're worried about hair loss

Exercise is an important lifestyle habit for maintaining muscle and cardiovascular health and metabolic function.

The key to male pattern hair loss lies in genetic predisposition and the androgen sensitivity of hair follicles, rather than in how much you exercise.

If you are worried about hair loss, the sensible approach is not to stop exercising but to identify the exact cause of your hair loss and combine adequate nutrition with appropriate treatment.
